Joint Forward Plan – How we will deliver our strategy from 2023 – 2028
The Joint Forward Plan sets out how the Integrated Care Board and its partners will contribute and deliver our strategy. We have created our Joint Forward Plan from a ‘bottom up’ approach – seeking to bring together in one place how all parts of our ICB are working together with partners to deliver our ambitions and commitments to meet the needs of local populations. Our plan, which we are publishing as a working draft, builds on the work undertaken to inform our Integrated Care Partnership Strategy and the Working with People and Communities: Engagement Strategy and will continue to develop as we build on our partnership work and understanding priorities for our populations. As we move into 2024/25, we have taken stock of our system maturity and have challenged ourselves to think through how we can make the highest impact changes for our populations. The second-year refresh of our Joint Forward Plan, which can be found by clicking this link, sets out how we plan our services to respond to the continuing rise of chronic ill-health and frailty, shifting the focus to preventing ill health and tackling the wider determinants of health.

Quality Assurance and Improvement Framework 2023 – 2026
This document describes the framework adopted by the Humber and North Yorkshire (HNY) ICB to deliver on our statutory duty for quality.
It sets out our vision for quality, the application of the National Quality Board (NQB) guidance, our governance arrangements and quality priorities. Additionally, it sets out the approach to driving quality improvement via the utilisation of our assurance processes as well as the Innovation, Research & Improvement System (IRIS) for HNY.
It is expected that the Quality Assurance Improvement Framework (QAIF) will be refreshed annually to support ongoing quality improvement and emerging concerns across the Integrated Care System (ICS). View Quality Assurance and Improvement Framework 2023 – 2026 here.
Engagement Approach 2024 – 2028. Working with People and Communities
This plan was developed in collaboration with all the partners that make up the Humber and North Yorkshire Integrated Care System, including engagement and patient experience professionals, people from voluntary and community groups, Healthwatch, as well as
patients and members of the public.
This plan was approved by the Humber and North Yorkshire Integrated Care Board on
9 October, 2024 and will be reviewed in 2028 – in line with the Joint Forward Plan.
Our original plan was launched in July 2022. We have refreshed this as we felt two years into our life as an organisation we are in a stronger position to identify meaningful aims and ambitions for the longer term.
We believe in keeping our patients and public informed about the latest local health and social care developments.
